These publicly funded services are provided through Ontario Health atHome. They will coordinate support such as:
- Nursing care (wound care, injections, chronic condition management)
- Personal support services (help with bathing, dressing, hygiene)
- Therapies (physiotherapy, occupational therapy, speech-language therapy)
- Nutrition and social work services
- Palliative and end-of-life care
- Help transitioning home after a hospital stay
